P344 Diverting ileostomy in Inflammatory Bowel Disease's surgery: the real clinical cost analysis

Abstract Background A loop ileostomy does not prevent an anastomotic leak and it is risk-free, but can decrease the clinical impact of dehiscence. Inflammatory bowel disease condition more complex than other benign situations candidated to colorectal surgery. The aim our study was assess risk complications related stoma presence identificate variables with in a series IBD’s patients undergoing various surgeries along primary diverting stoma. Methods We conducted retrospective who underwent sphincter-preserving elective surgery for IBD between January 2014 December 2020 at IRCCS Sacro Cuore Don Calabria Hospital. Data were collected regarding baseline demographics, operative techniques complications. Ileostomy-related separated into three categories: creation time index within 30 days; having ileostomy; closure. Results Fifty-six ileostomies performed included (table 1). Seventeen (30%) procedures had following complications: 5 or pelvic abscess (8.9%), 3 obstruction (5.4%) 7 (12.5%) bleeding requiring transfusion. Two creation: 1 prolapse (1.8%) rotation (1.8%). Seven required reoperation. readmitted days after dehydratation. Twenty-four (42.1%) suffered during same from management’s problems. Eighteen (32.1%) closure 6 (10.7%), SSI (12.5%), (8.9%) 2 transfusions (3.6%). Nine (16%) At univariate analysis status are frequent male (p=0.01) (p=0.04). Ileostomy common (p=0.005). Complications found multivariate statistically maintenance. Conclusion alone cannot mitigate overall high rate expected patients, also consideration potential problems it-self related. Even if presents small court, when resection necessary despite patient's conditions we propose resect without anastomosis procedure restore continuity only optimization.
